Mrs KIRKLEY was admitted to hospital following a number of falls at the care home where she was resident which resulted in a number of rib fractures. She developed an infection from which she did not recover. Evidence was heard that Mrs KIRKLEY had become increasingly frail and suffered from mixed dementia which contributed to the falls occurring |

The pneumonia was experienced against pre-existing conditions of occupationally caused diffuse pleural thickening and coronary artery disease, which contributed to his death by making him more susceptible to the effects of the pneumonia. |

| 22/09/2026 | 22/09/2026 | HURTLEY, William | Mr William HURTLEY died on the 17th December 2025 in Preston hospital from chest sepsis and hypertensive heart disease. Mr HURTLEY was admitted to hospital on the 1st December 2026 with a chest infection and heart failure and during his admission it was suspected that he was suffering from a pulmonary embolism. A CT Pulmonary angiogram ruled out a pulmonary embolism but despite this Mr HURTLEY continued to receive a treatment dose of anticoagulant for another two days which caused him to suffer a retroperitoneal bleed. Despite treatment to reverse the effects and confirmation that there was no longer any active bleeding, the haemorrhage affected his physiological reserve and made him more susceptible to infection and contributed to his death. |

Natasha was found submerged in the bath where a sponge was covering the plug and she died from drowning. At the time of her death she was acutely intoxicated. Natasha was experiencing significant challenging personal issues at the time of her death and was being supported by professionals. Her intentions cannot be determined. |
| 25/09/2026 | 29/09/2026 | BARCLAY, Robert | Robert BARCLAY died on the 25th March 2026 at the St Catherine's Hospice, Lostock Lane, Preston. Mr BARCLAY had been diagnosed with adenocarcinoma of the stomach, for which he underwent a robotic gastrectomy surgery on the 25th February 2026. Following the surgery, Mr BARCLAY initially made a steady recovery. On the 3rd March 2026, Mr BARCLAY's observations required further monitoring and he underwent a chest X-ray and ECG which suggested possible hospital acquired pneumonia. At around 03:40am on the 4th March 2026, Mr BARCLAY was found unresponsive in his bathroom. He entered cardiac arrest and was resuscitated with a return of circulation in 8 minutes and 16 seconds. Investigations were not able to identify the cause of Mr BARCLAY's cardiac arrest but did confirm that he had suffered a hypoxic brain injury as a consequence of the arrest. His care was reorientated to palliative and he was transferred to the hospice where he died as consequence of hypoxic brain injury caused by cardiac arrest. His underlying cancer and surgery contributed to his death occurring. |

Mrs DANVERS sustained a small subdural haemorrhage and neck injury when she fell in her home on the 27th April 2026. She was taken to hospital and her apixaban medication was withheld and she was for conservative management of her injuries and was discharged home the same day. She deteriorated unexpectedly on the 28th April 2026 due a stroke and care to support her comfort and dignity was provided. Her death was contributed to by the injuries she sustained in the recent fall as well as old age, osteoporosis and her apixaban being withheld. |

| 28/09/2026 | 28/09/2026 | MAUDSLEY-DEWHURST, Arlo James | Arlo James MAUDSLEY-DEWHURST was born on the 28th July 2025 at the Burnley General Hospital. His mum had been induced early due to growth scans showing Arlo to be a big baby. Once labour had started, Arlo's mum experienced very quick contractions and later developed urgent, significant bleeding. She underwent an emergency Caesarean section to deliver Arlo, who was very unwell at birth. Arlo's placenta showed velamentous cord insertion which was the cause of his mum's haemorrhage. Despite treatment which included hypothermic cooling, Arlo did not recover and his care was reorientated to palliative care. He died in the arms of his parents on the 31st July 2025 as a consequence of multiple organ failure caused by hypoxic ischaemic encephalopathy as a result of velamentous cord insertion. |
| 28/09/2026 | 29/09/2026 | WHITE, Janet Kathleen | Janet Kathleen WHITE died on the 11th September 2025 at Royal Bolton Hospital from multi-organ failure caused by sepsis which she developed following an operated right hip fracture. Ms WHITE had a lengthy hospital admission prior to her death which was complicated by a fall on the 17th June 2025 through which she suffered a fractured neck of femur requiring surgery which was performed on the 21st June 2025. Ms WHITE then suffered a dislocation requiring surgery on the 25th July 2025. Ms WHITE deteriorated and despite treatment to manage an infection, she continued to deteriorate and care was orientated to palliation before her death on the 11th September 2025. Ms WHITE had been suffering an acute deterioration in her mental health since earlier in 2025, for which she had been receiving treatment as an inpatient. |

Mr WIDDOWS had significant underlying respiratory diseases but had also been exposed to asbestos in the course of his previous working life. His death was caused by underlying respiratory conditions but was contributed to by asbestos exposure in his previous working life. |
