14/04/2026 14/04/2026 DUNN, Valerie Dorothy Mrs Valerie DUNN died on the 9th August 2024 in Preston Hospital when she suffered an out of hospital cardiac arrest caused by a pulmonary embolism as a result of a deep vein thrombosis. Mrs DUNN underwent total right hip replacement surgery in June 2024 and attended the Urgent Care Centre on the 3rd August 2024 where she was prescribed apixaban and a CT Pulmonary Angiogram was ordered to confirm or exclude a pulmonary embolism. On attendance at the appointment to undergo the CT Pulmonary Angiogram on the 5th August 2024, there was a failure to note her raised troponin level, resulting in her being incorrectly advised to stop her anticoagulation treatment and the CTPA was not completed. Following discharge she collapsed on the 9th August 2024 and was taken to hospital and despite efforts she was unable to be resuscitated. Had the CT Pulmonary Angiogram been carried out as planned, it would likely have resulted in the diagnosis of a pulmonary embolism and the continuation of anticoagulation and Mrs DUNN would not have died when she did.
14/04/2026 14/04/2026 HUGHES , Michael Joseph Michael Joseph HUGHES died on 1st January 2026 at Royal Preston Hospital from septic shock. Mr HUGHES developed an infection following successful surgery to treat a severe traumatic head injury sustained on 25th December 2025 when he fell down a flight stairs. Despite ongoing maximal treatment in hospital he continued to deteriorate.

16/04/2026 16/04/2026 CRITCHLEY, Ian Andrew Ian CRITCHLEY died on the 9th November 2023 in Chorley Hospital, as a result of fatal hypoglycaemia. Mr CRITCHLEY, who had type 2 diabetes mellitus, was admitted to hospital with sepsis on the 31 October 2023 which was treated however he was then suffering with hyperglycaemia and hyperkalaemia and commenced on a variable rate insulin infusion on the 8th November 2023 to correct this. His blood sugars should have been monitored hourly in line with policy but no blood sugar checks were completed and the infusion remained running without any adjustments being made to the treatment. Mr CRITCHLEY was found unresponsive and in cardiac arrest the following morning and could not be resuscitated. The failure to monitor his blood sugar levels for a period of 6 hours contributed to his death. His death was contributed to by neglect.

21/04/2026 21/04/2026 AINSWORTH, Kiara Samantha Kiara Samantha AINSWORTH died on the 27 April 2025 in her home in Accrington, when she suffered an epileptic seizure whilst unsupervised in the bath which caused her to drown. She had not received three doses her of epileptic medication which likely contributed to her experiencing a seizure. At the moment she suffered the seizure she was not being supervised in the bath. 
21/04/2026 21/04/2026 GUNTON, Jasper Jasper GUNTON was born and died on the 21st December 2023 at his home address. He was born following a pregnancy which had been concealed and had not benefitted from ante-natal care. After his birth, Jasper did not receive medical attention and he was taken to bed with his parent who fell asleep with baby Jasper on their chest. In the morning, Jasper was found unresponsive and despite resuscitation attempts by paramedics, was pronounced dead. It is not possible to determine what caused Jasper's death which occurred in the context of a pregnancy and birth without medical assistance. 
21/04/2026 21/04/2026 HUNT, Vincent John Vincent John HUNT died on the 10th January 2025 at the Royal Preston Hospital as a consequence of blood loss. Mr HUNT was an inpatient for the purposes of receiving dialysis due to acute kidney injury, chronic kidney disease and a background of multiple myeloma. Mr HUNT was staying in a side room due to infection. On the evening of the 10th January 2025 Mr HUNT was found unresponsive in a pool of blood. A femoral line which had been inserted to facilitate dialysis was dislodged, resulting in the blood loss. It is not possible to determine how the line became dislodged.

23/04/2026 23/04/2026 McCLUGGAGE, Gary Gary MCCLUGGAGE died on the 11th May 2025 at the Royal Blackburn Hospital as a consequence of decompensated heart failure. Mr MCCLUGGAGE had a number of health conditions and lived in sheltered accommodation. He had not been seen by staff since late April 2025. He was taken to hospital on the 6th May 2025 when he was found after intervention by his brother to be very unwell and stuck in his chair for a number of days. He was taken to hospital where despite treatment he died.

27/04/2026 28/04/2026 SHAW, Julie Julie SHAW died on the 28th October 2025 at her home address. Ms Shaw suffered a ruptured abdominal aortic aneurysm in February 2025 which was operated upon. Following the surgery she suffered a number of complications which resulted in loss of blood flow to her bowel and required many more surgeries. In the months that followed, Ms SHAW endured a long hospital stay where she struggled to manage adequate oral intake and was told she was not eligible for inpatient mental health support. She was discharged with a package of care but her family provided the majority of her care. Ms SHAW had been unable to maintain adequate oral intake for many months and her health continued to deteriorate. She received support from palliative care services before her death.
28/04/2026 28/04/2026 BAKER, Marjorie Marjorie BAKER died on the 8th January 2026 at the Chorley and South Ribble General Hospital as a consequence of sepsis caused by a sacral ulcer. Mrs BAKER had a number of underlying health conditions and lived in a care home. She developed a pressure injury to her sacrum which became infected and caused her death.
